About the Collaborative Planning and
Implementation (CPI) Initiative

The CPI initiative provides funding for regional collaborative planning efforts to support the implementation of Enhanced Care Management (ECM) and Community Supports. 

The Santa Cruz County CPI group is composed of stakeholders that work together to identify, discuss, and resolve topical implementation issues, and share best practices. We identify how Providing Access and Transforming Health (PATH) and other CalAIM funding initiatives – including the Technical Assistance Marketplace – may be used to identify and address implementation and sustainability needs and gaps.

As the facilitator selected for the Santa Cruz County PATH Collaborative, HIP hosts and facilitates convenings, including our monthly Collaborative meetings as well as the Enhanced Care Management and Community Supports workgroup meetings. HIP works with participants to develop programmatic activities contributing to CalAIM’s whole system, a person-centered approach to care.

Get to know the PATH Team

HIP is a trusted advisor in Santa Cruz County (and beyond) and is deeply connected to our local community, having worked with many healthcare organizations, CBOs and our MCP over the years. This unique connection, relationship and knowledge of our community strengthens our important role as neutral conveners in our community among public and private healthcare leaders. Our reputation in the community is that we “make things happen”, and we love that about us.

Lunch & Learn Series

HIP conducted two three-part series over the noon hour in October to use quality improvement (QI) principles, and in December to cover the basics of CalAIM, the PATH Initiative, Enhanced Care Management, Community Supports, and other Medi-Cal programs.
